Transportation Options for Seniors (TOPS) Program
...I never would have believed that in a short time I would be in a wheelchair. It seems simple that someone with good friends would have no trouble arranging a trip to the doctor, but it is - for all kinds of reasons - all of which I understand. Thank goodness for Senior Services North Fulton and I thank those who kindness provides it. Please know that each trip you arrange is a real gift. I sincerely appreciate this wonderful service - it is SO important.
Transportation Provided by: Destiny Transportation, Five Star Transportation, Synergy HomeCare, each under contract to SSNF
Who is eligible: For elderly seniors and those with significant vision or other physical limitations, the service can include a One on One service feature, whereby the senior is not only transported to the appointment, but has someone stay with them to assist with note taking, paperwork and so forth. The One on One feature is currently not available for seniors who need an electric wheelchair lift. Destinations: Medical appointments and physical therapy
Frequency: Up to 3 trips per month unless approved by Care Manager for more
Lead Time: Minimum of 48 hours notice prior to appointment
Wheelchair Accessible: Yes
Cost: A voluntary contribution of $5.00 one way or $10.00 per round trip, to help fund the program, is invited - but ability to contribute is not a requirement to receive this service.

Transportation for SSNF Senior Center Participants
Provided by: Fulton County
Who is eligible: Senior Center participants, age 60+
Destinations: To SSNF Neighborhood Senior Centers and back, Monday-Friday. Depending on individual Center programming, transportation is provided for shopping trips, activities and events
Wheelchair Accessible: Yes. Participants must be ambulatory with or without an assistive device
Cost: Donations are accepted for the Center programs at each Neighborhood Senior Center

Who is eligible: Medicare recipients, age 60+
Destinations: Medical appointments
Frequency: Subscription service available for specific times, i.e. chemotherapy, 3x/week for dialysis or 2x/month for other medical appointments
Lead Time: 2 - 4 weeks in advance

Jewish Escorted Transportation Services (JETS) For Seniors in North Fulton

Who is eligible: Age 60+
Service Area: Zip codes 30075 and some of 30076, within a 12 mile radius of the JETS Facility in Dunwoody
Destinations: Medical appointments. Other types of requests will be considered based on availability
Wheelchair Accessible: Yes
Cost: Within 12 miles: $11.00—$12.00 each way; with wheelchair and escort, $20.00 each way. Beyond 12 miles, an additional fee applies

MARTA ParaTransit For Seniors in North Fulton
Who is eligible: Persons with disabilities who are, because of their disability, unable to board, ride or disembark from an accessible vehicle on MARTA’s regular bus or rail service
Service Area: Within Fulton and Dekalb counties along a 3/4 mile corridor located on each side of all fixed bus routes and within a 3/4 of a mile radius of each station
Wheelchair Accessible: Special lift-equipped vans are provided on a curb-to-curb, shared ride basis
Cost: $3.50 each way. Personal attendant travels free
