Senior Services North Fulton

Under the direction of an MSW Social Work Professional, our Care Managers conduct home visits with seniors to determine which services best meet their needs.   SSNF has contracted with area providers of in-home services and pays those providers directly.  That enables the senior to receive the services without having to pay the care provider or worry about whether they can afford to pay for the assistance that they need.   Although SSNF is not providing the in-home assistance with our own staff, we continue to remain involved with the senior by providing comprehensive management of care.  That gives the senior, and their family, confidence that only needed services will be provided.  SSNF continues to monitor the care received and to reassess the senior’s needs throughout the year, in case situations change and need to be reflected in the care management plan. We also survey our seniors to assess whether the quality of service and the relationship between the senior and their care giver is meeting expectations.
